CERES5.01.90 - Phone Ext. No. update on Agency and Vendor cards

Issue Addressed: Phone Ext. No. field is not on the Agency page. The Phone Ext. No. field appears twice on the Vendor Card - Address & Contact Tab.

This release does not present a change in system usage methodology. 

Resolution: Ceres was enhanced by adding the Phone Ext. No. control to the Agency Card on the Address & Contact tab and removing the duplicate Phone Ext. No. control from the Vendor Card.
