Issue addressed: When the Agency Activity Review Matrix page (PG 14012738) is customized by the user to hide some unwanted non survey specific fields (i.e. FBC fields or Parent Agency No.), when the page is re-opened the survey text fields are hidden. Using "Restore Defaults" does not restore the survey text fields on the page. The only way to return the page to the original state is to remove the "User Metadata" record for the page/user.

This release does not present a change in system usage methodology.
Resolution: The Agency Activity Review Matrix page was updated by removing an extra group on the page and incorporating the fields under the remaining group.
